What the Medicare Drug Price Negotiation Program means for your pharmacy
The Inflation Reduction Act (IRA) authorizes Medicare to negotiate prices on selected high-expenditure, single-source drugs under Part D, with negotiated Maximum Fair Prices (MFPs) taking effect beginning of this year [2026]. For community pharmacies, this means you will dispense selected drugs at the negotiated MFP while manufacturers are responsible for making you whole through rebates.
Key implications for independent pharmacies:
- Selected drugs will have a Medicare-negotiated Maximum Fair Price (MFP) starting with the first group of 10 drugs in 2026, with additional negotiation cycles in subsequent years [15 in 2027].
- You will continue to submit claims as usual, but reconciliation of MFP-related payments and refunds are being run through two new platforms: the Medicare Transaction Facilitator and Beacon.
- Pharmacies must be enrolled and configured correctly in both systems to prevent delayed payments, missing refunds or unresolved disputes.

Overview of the Medicare Transaction Facilitator (MTF)
The Medicare Transaction Facilitator (MTF) is a free, secure web-based system that supports the implementation of the Medicare Drug Price Negotiation Program. It facilitates secure data exchange between CMS, manufacturers and dispensing entities so manufacturers know which pharmacies dispensed MFP-eligible prescriptions and what refunds are owed.
For pharmacies, the MTF allows you to:
- Enroll your locations as participating dispensing entities under the MDPNP.
- Express your preference for how you receive MFP-related payments (e.g., direct from manufacturers or via other arrangements).
- MTF enrollment keeps your pharmacy compliant with Med D plan contract requirements.

What is Beacon and how does it support MFP refunds?
Beacon is an innovative technology platform, provided at no cost by participating manufacturers, that processes MFP rebate and refund requests under the Medicare Drug Price Negotiation Program. It is separate from the MTF but works in tandem: MTF facilitates data exchange and program enrollment, while Beacon is the primary tool pharmacies use to track, reconcile and question MFP-related payments.
Beacon enables pharmacies to:
- Register as a dispensing entity and associate all eligible locations through your NCPDP Relationship ID.
- View the status of MFP rebate requests for eligible prescriptions dispensed by your pharmacy.
- Review detailed pricing information on each MFP refund, including how the refund amount was calculated.
- Initiate good faith inquiries (GFIs) when you believe an MFP refund is incorrect or missing.
