Surescripts Class Action Settlement

Who Is Eligible?
Any pharmacy located in the United States or its territories that paid for e-prescriptions routed through the Surescripts network from September 21, 2010 through July 18, 2025 is eligible to submit a claim. Exclusions include employees, owners, and affiliates of the defendant companies, as well as government entities and legal representatives connected with the case.

How To File Your Claim
To file a claim:
- Visit www.surescriptsantitrustlitigation.com and complete the online form or request a paper claim.
- Provide your pharmacy’s full legal name, contact information, tax ID (last 4 digits), and documentation of qualifying Surescripts e-prescription payments during the eligible period.
- Ensure all information is accurate and submitted before November 28, 2025.
- If you filed during the RelayHealth phase, you may supplement with additional information for transactions between April 20, 2021, and July 18, 2025.
