Smith Pharmacy: Independent Roots, Stronger with IPC

In December of 2013, Nic and Nikki Smith took a leap of faith that would change not only their lives but also the healthcare landscape of their community. After a decade working in chain retail pharmacy, Nic had grown frustrated with corporate red tape and decision makers who weren’t in the trenches with patients. Too much time was spent checking boxes instead of caring for people. So, the couple mortgaged their house, stepped into the world of independent ownership and opened the doors to Smith Pharmacy.
The early days were rough. Pharmacy itself was the easy part, but payroll, HR, accounting, and building maintenance often stretched late into the night. Nic recalls learning how to repair equipment and navigate QuickBooks through YouTube tutorials, all while filling prescriptions during the day. Those first years ran in the red, but they persevered, eventually hiring accountants, pharmacists, and staff who gave Nic the freedom to focus fully on practicing pharmacy.
Today, Smith Pharmacy has grown far beyond its humble beginnings. What started in 1,500 square feet has expanded several times, now preparing to move into a brand-new 23,000 square foot facility, with plans to add future sterile compounding space. The result is a culture where patients don’t just stop in for medicine; they linger, talk with neighbors, and connect. As Hope Smith, who manages marketing, compliance, and business operations, explains, “We try to make it an experience so coming here is not a chore.”

For Nic, independence means freedom to innovate. He regularly hosts education sessions for physicians and patients on compounding, CBD, over-the-counter therapies, and insurance navigation. He emails local doctors with pharmacy updates, such as new generics or changes in drug availability, to ensure everyone stays informed.
That proactive approach has led to countless stories of impact. Recently, a mother drove two hours after every other pharmacy turned her away when she needed compounded oral suspensions for her child newly diagnosed with leukemia. Smith Pharmacy not only made the medications on short notice but also worked to ensure Medicaid coverage. “The ability to make a bad situation a little bit better is very rewarding,” Nic reflects.

Like every independent, Smith Pharmacy faces shrinking PBM reimbursements and razor-thin margins on brand medications. The current reimbursement model is unsustainable, Nic says, especially when vital therapies like GLP-1s can cost over $1,000 while the system leaves pharmacies struggling to cover costs. To survive, independents must be adaptive.
Smith Pharmacy has leaned heavily on compounding, long-term care services, and even developing self-funded employer plans that function like PBMs but with transparency and fairness. Advocacy also plays a role. Nic is active in legislative efforts, working with Wisconsin lawmakers to push for PBM reform and more pharmacy-friendly policies.
