Back in January, independent pharmacy owners were preparing for a year filled with uncertainty. Reimbursement changes, cash flow concerns, and evolving purchasing strategies were top of mind. Six months later, we have moved past the predictions and into reality.
The good news is that many pharmacy owners have shown incredible resilience. They’ve adapted, adjusted, and found ways to continue serving their patients despite ongoing challenges. At the same time, the first half of the year has reinforced an important lesson: pharmacies that stay proactive are often in a stronger position than those simply reacting to what happens around them.
As we move into the second half of 2026, the focus shifts from preparation to execution. What are pharmacy owners experiencing right now? What opportunities exist? And where should owners focus their attention for the remainder of the year?

What Are We Seeing in the Marketplace?
When we look across our conversations with pharmacy owners, many of the trends we expected at the start of the year have come to fruition.
Cash flow remains one of the biggest topics. While some pharmacies are still generating strong profits, timing continues to be a challenge. Owners are paying for inventory today while waiting for reimbursement and rebate dollars to catch up. That puts increased pressure on working capital and requires a higher level of financial awareness than many pharmacies have needed in the past.
We’re also seeing pharmacy owners become much more intentional about purchasing decisions. Instead of simply focusing on acquisition cost, owners are evaluating how purchasing strategies impact compliance, rebates, gross profit, and overall cash flow.
At the same time, pharmacies continue to face many of the same pressures that have existed for years. Staffing remains challenging. Reimbursement remains unpredictable. Yet despite these headwinds, we are seeing many pharmacies maintain strong performance because they are paying close attention to their numbers and making data-driven decisions.
The pharmacies that are doing well are not necessarily the largest or busiest. In many cases, they are the owners who understand their business, monitor key metrics regularly, and adjust quickly when conditions change.
What Questions Are Pharmacy Owners Asking Right Now?
If there is one thing that has stood out this year, it’s that pharmacy owners want clarity.
Many of our conversations begin with questions like:
Am I buying the right way?
Is my generic compliance where it should be?
How does my current deal compare to other opportunities?
Am I maximizing available rebates and incentives?
Why does my cash flow feel tighter even when prescription volume is steady?
What should I be focusing on right now?
These are smart questions because they get to the heart of pharmacy profitability.
Owners are realizing that success isn’t driven by a single metric. A pharmacy can have great prescription volume and still struggle financially. Likewise, a pharmacy may identify significant opportunities simply by improving compliance, optimizing inventory, or adjusting purchasing behaviors.
One thing we consistently tell owners is that they don’t have to figure everything out on their own. The independent pharmacy landscape is changing quickly, and sometimes an outside perspective can help identify opportunities that are difficult to see from inside the day-to-day operation.
How Is the Member Performance Team Helping Pharmacies?
One of the biggest misconceptions we encounter is that Member Performance is focused on only one area of the business. In reality, our team works alongside pharmacy owners to help them improve financial performance, evaluate purchasing strategies, identify operational opportunities, and make more informed business decisions.
Our team supports pharmacies through financial reviews, deal comparisons, compliance tools, EDI ordering support, group ordering opportunities, and warehouse optimization strategies. We also collaborate closely with account managers, wholesaler partners, and pharmacy system providers to help pharmacies maximize the value of the programs and resources available to them.
The goal is simple: provide pharmacy owners with actionable insights backed by data.
Whether we’re helping an owner better understand their financial performance, evaluating purchasing patterns, reviewing compliance metrics, or identifying opportunities to improve margins, the focus is always on delivering practical solutions that can make a measurable impact.
What makes our team valuable is not just the tools we provide, but the perspective we bring. We work with pharmacies across the country and see what successful operators are doing. That allows us to help pharmacy owners identify opportunities, avoid common pitfalls, and make decisions with greater confidence.
Ultimately, our role is not to tell owners how to run their business. Our role is to provide information, analysis, and guidance that helps them make the best decisions for their pharmacy.
Where Are the Opportunities for the Rest of 2026?
Despite the ongoing challenges, there are still meaningful opportunities available for independent pharmacies.
One area continues to be purchasing optimization. Small improvements in compliance, buying behavior, and inventory management can have a measurable impact over time. Owners who understand where their products are coming from, how they affect rebate performance, and how they impact cash flow often place themselves in a stronger position than those who only focus on invoice price.
Technology is another area of opportunity. Whether through EDI ordering tools, pharmacy system integrations, or reporting resources, pharmacies have more data available than ever before. The challenge is turning that information into action.
Financial planning also deserves attention. The pharmacies that successfully navigate changing reimbursement environments are often the ones that forecast ahead, understand their working capital needs, and proactively address issues before they become major problems.
The opportunities are there. The key is identifying which opportunities make the most sense for your specific pharmacy and creating a plan to take advantage of them.
What Should Pharmacy Owners Focus on Between Now and the End of the Year?
If I could sit down with every independent pharmacy owner today, I would encourage them to focus on understanding their business at a deeper level than ever before. The remainder of 2026 is not about making drastic changes overnight. It’s about making informed decisions and staying disciplined in the areas that have the greatest impact on long-term success.
Take the time to review your financial performance regularly and understand what is driving profitability in your pharmacy. Evaluate your purchasing strategy and make sure it aligns with your business goals. Pay close attention to compliance metrics, inventory levels, and cash flow trends so you can identify opportunities or concerns before they become larger issues.
Most importantly, don’t wait until something goes wrong to ask questions. The pharmacy owners who tend to perform best are those who remain proactive, seek out information, and take advantage of the resources available to them. Small adjustments made today can have a meaningful impact on performance by the end of the year.
Independent pharmacy continues to face challenges, but it also continues to create opportunities for owners who are willing to adapt, stay engaged, and focus on the fundamentals. The second half of the year is an opportunity to strengthen your business, improve efficiency, and position your pharmacy for long-term success.
